Insurance – YOU DO NOT NEED TO SUBMIT PROOF OF INSURANCE - you are indicating that you have explored and obtained appropriate insurance for your business type, which can be different from another member in our association or person practicing in any holistic modality, including one similar to your own. You understand that the CFA is not responsible for claims made against you, your failure to obtain insurance or appropriate insurance, even if you are following advise or recommendations provided by the CFA or a CFA recognized school. You also understand that by submitting payment of your membership fees and checking this box you are assuring the CFA that you have insurance. Finally, you understand that if you do not agree to these statements and have checked the box anyways, you are misrepresenting your business to the CFA.
Code of Ethics – you have read, understand and will follow the guidelines provided in the Code of Ethics.
Scope of Practice – you have read, understand and will follow the guidelines provided in the Scope of Practice.
Harassment – you have read and understand the Anti-Harassment Policy. The CFA strongly encourages members to obtain Harassment training annually. Please note Harassment training courses will count as a primary CEU.
Human Rights – you understand the Human Rights requirements in your area, province, country, etc. The CFA strongly encourages members to obtain training in Human Rights annually. Please note Human Rights training courses will count as a primary CEU.
Records Management – you understand you are responsible for retaining the necessary insurance and records for membership at all times. The CFA will randomly audit members to ensure compliance with membership requirements. By checking these boxes, you understand that if you are requested to provide a document you agree to submit it willfully and respectfully. Failure to maintain membership could be grounds for immediate termination without a refund of fees.
